결막에 발생한 악성 흑색종의 보존적 치료 1 례

결막 악성 흑색종은 눈에 발생하는 악성 흑색종의 약 2%, 악성 종양의 1%이하를 차지하는 드문 종양으로 대개 원발 후발성 흑색증(Primary acquired melanosis)에서 발생하며 적절한 치료를 받지 않을 경우 처음에는 림프관을 통해 주변 림프절로, 결국에는 신체의 다른 부위로 전이하게 된다. 최근에는 많이 진행되어 절제 불가능한 경우를 제외하고는 보존적으로 종양을 제거한 후 냉동 치료를 하는 방법이 선호되고 있다. 저자들은 재발된 결막 악성 흑색종을 주소로 내원한 환자에서 보존적 방법으로 치료한 경우를 경험하였기에 문헌 고찰과 함께 보고하는 바이다(한안지 41:1012~1016, 2000). Primary malignant melanoma of the conjunctiva represents less than 2% of ocular melanomas and less than 1% of malignant tumors of the eye and usually derives from PAM(Primary acquired melanosis). Conjunctival melanoma spreads initially via lymphatics to the regional lymph nodes and eventually to the other organs in the body. Recently, except for advanced unresectable ases, conjunctival melanoma is treated by excision of the mass combined with cryotherapy. We report a case of conservative treatment in a patient who presented recurred conjunctival melanoma, along with the review of literatures(J Korean Ophthalmol Soc 41:1012~1016, 2000).
