정상안압 녹내장에서의 전신적 위험요소에 대한 분석

시신경유두의 허혈상태가 정상안압 녹내장의 유발요인중 하나일 가능성에 대하여서는 많은 보고가 있어 왔고 최근에는 이를 기초로 한 녹내장 치료약제에 대한 연구가 활발히 진행되고 있다. 하지만 이와같은 시신경 유두의 허혈상태,나아가서는 정상안압 녹내장이 전신적,특히 심혈관계의 질환 유무와 어떤 관계를 가지고 있는지를 명확히 규명하지 못하고 있는 상태이다. 또한 한국인에 있어 이와같은 정상안압녹내장이 다른 민족보다 높은 유병률을 나타내고 있음이 알려져 있지만 아직까지 한국인에 있어 정상안압녹내장과 전신적,특히 심혈관계의 이상 유무와의 관계는 보고된 바 없다. 이에 대해 저자들은 한국인에 있어 정상안압 녹내장 환자와 정상 대조군의 혈액점성 인자,혈액응고 인자,그리고 생화학적 인자를 비교 분석해 보았다.정상안압녹내장으로 진단받은 30명의 환자를 대상으로 이를 정상 대조군 30명의 검사결과와 비교 분석하였다.정상안압 녹내장 환자군에서는 정상 대조군의 결과에 비하여 높은 총 콜레스테롤 수치와 저밀도 지단백 콜레스테롤수치를 나타내었으며 이는 통계적으로 유의하였다. 그러나 고밀도 지단백 콜레스테롤수치는 정상대조군에 비하여 낮은 수치를 나타내었으나 이는 통계적으로 유의하지 않았다. 또한 혈액점성인자,혈액응고 인자는 두 군에서 차이를 보이지 않았다.한 국인의 정상안압 녹내장에 대한 이와같은 분석결과는 전신적인 고콜레스테롤혈증이 시신경유두의 혈액순환에 장애를 일으키고 이러한 혈관관류압의 장애가 정상안압 녹내장을 유발하는 중요한 인자일 가능성을 제시해 주고 있다. It has been suggested that normal tension glaucoma could be the result of pressure independent factors.One of the current theories is that normal tension glaucoma is the result of decreased optic nerve perfusion on the basis of vascular disease or other systemic factors such as altered blood viscosity or coagulation state.This study compared the vascular viscosity profiles,coagulation tests,and biochemical tests of 30 consecutive cases of normal tension glaucoma with 30 control cases to evaluate association of particular systemic state with normal tension glaucoma in Korean population. The data of total cholesterol and low density lipoprotein(LDL)cholesterol were significantly higher in the normal tension glaucoma group than in the control group.But there were no significant differences between the two groups with respect to the many other factors examined.These results support the vascular ischmic theory due to hyperlipidemia in the pathogenesis of the normal tension glaucoma.Inadequate circulation of optic nerve due to atherosclerotic change might play an important role in the pathogenesis of normal tension glaucoma.(J Korean Ophthalmol Soc 40:2239~2244,1999).
