단순포진 각막포도막염의 임상양상

저자들은 단순포진 각막포도막염 환자들을 대상으로 이 질환의 임상양상에 대해 알아보았다. 1987년 5월부터 1998년 1월까지 한양대학교 병원에서 단순포진 각막포도막염으로 진단된 28명의 환자를 대상으로 후향적 조사를 실시하였다. 28명(남자 19명, 여자, 9명)의 평균 추적관찰기간은 14.0개월이었다. 자세한 병력청취가 불가능했던 7명을 제외한 21명 중 5명에서 처음 포도막염이 나타났으며 16명에서는 재발성 포도막염이었다. 각막병변은 실질 각막염(간질 각막염, 원반 각막염, 괴사성 각막염)이 23안으로 가장 많이 동반되었으며, 상피 각막염(수지상 각막염, 지도상 궤양, 변형포진 각막염)이 7안, 각막 내피염이 3안에서 관찰되었다. 안압은 21㎜Hg 이상이 10안에서 관찰되었으며, 이중 5안에서 30㎜Hg 이상이었고 이는 모두 재발성 환자에서 나타났다. 홍채후 유착은 9안에서, 홍채우축은 4안에서 나타났다. 단순포진각막포도막염은 단순포진 실질각막염에서 흔히 동반되며, 재발을 잘하고 안압상승, 홍채후유착, 홍채위축, 전방축농 등의 소견을 보일 수 있으므로 계속적인 관찰이 필요하리라 생각된다. (한안지 39:2298∼2302, 1998) A retrospective study was done to analyze the clinical characteristics about herpes simplex keratouveitis. We reviewed medical records of 28 patients who had been diagnosed as having herpes simplex keratouveitis at Hanyang University Hospital from May 1987 to January 1998. Mean follow-up period was 14 months. In 28 patients(male 19, female 9), first attack was in 5 patients, recurrent uveitis in 16 patients, and 7 patients were undetermined due to lack of detailed clinical history, Stromal keratitis(intersititial keratitis, kisciform keratitis, and necrotizing keratitis) was present in 23 eyes, superficial keratitis(dendritic keratitis, geographic eyes. Ten eyes had an increased intraocular pressure of 21㎜Hg or more, with 5 eyes having 31㎜Hg or more, which were all in cases of recurrence. Posterior synechiae were present in 9 eyes, iris atrophy in 4 eyes. Herpes simplex keratouveitis is prone to recur and frequently associated with herpes simplex keratitis, especially stromal keratitis. It may be associated with increased intraocular pressure, osterior synechiae, iris atrophy, or hypopyon, Therefore it is necessary to follow up persistently(J Korean Ophthalmol Soc 39:2298∼2302, 1998)
