위아절제술 환자에서 상부위장관 내시경으로 진단한 공장-위 장중첩증 1예

공장-위 장중첩증은 위수술 후 발생하는 비교적 드문 합병증으로 조기진단을 통해 즉각적으로 수술적 치료가 이루어지지 않으면 사망률이 높은 질환이다. 8년 전 위암으로 위아전절제술을 시행받은 78세 여자 환자가 내원 당일 발생한 심와부 통증과 구토 등의 증상을 주소로 내원하였다. 상부위장관 내시경을 통해 비교적 조기에 급성 공장-위 장중첩증을 진단하고 수술적 치료로 호전된 증례를 경험하였기에 문헌고찰과 함께 보고한다. Jejunogastric intussusception is a rare, but potentially lethal complication after gastrectomy or gastrojejunostomy. In the acute condition, early diagnosis and prompt surgical treatment are mandatory to reduce the incidence of mortality. We present here a case of jejunogastric intussusception that was diagnosed by gastroscopy in a patient with a history of subtotal gastrectomy, and she had experienced increasing epigastric pain and vomiting for 1 day.
