Zusammenfassung: | Lactic Acidosis frequently accompanies major motor seizures. This is of minimal clinical significance in most circumstances because the lactate is rapidly metabol- ized following the seizures. Acidosis is transient and is generally not appreciated because serum pH, bicarbon- ate, and lactate levels are not measured in most instances. Althogh this transient acidosis is of minimal importance in otherwise healthy individuals, the in- cremental acidosis of a grand-mal seizure may be important factor in the already ill patients with incipi- ent type A lactic acidosis. We have experienced a case of lactic acidosis after grand-mal seizure in a 29 years old man who had been admitted for acute renal failure due to systemic blunt truma. Immediately after seizure venous lactate concentra- tion was 14.6 mmol/L at first attack, 12 mmol/L at second attack. And arterial blood gas analysis showed pH 6.82, 6.98, HCO, 3.6meq/L, 8.6meq/L. and by 120 minutes after seizures, spontaneous resolution of lactic acidosis occurred. There was no change in serum potassium concentra- tion despite the development of a severe systemic aci- dosis and the subsequent return to previous levels of the H. So we present a case with the brief review of litera- ture. |
