Zusammenfassung: | Background/Aims: The poor results of surgery for gallbladder cancers are at least partially due to the inadequate setting of resection to the spreading characteristics of the tumor. As our result from the extended resection of advanced gallbladder cancers was favorable, we assessed the effectiveness of extended resection by analysis of tumor factors and surgical procedures. Methods: Records of 99 patients with gallbladder cancer who underwent surgery between 1989 and 1996 were reviewed according to the TNM staging system and the extent of resection. Results: The 1 and 2-year survival rates of patients with advanced gallbladder cancers following extended or conventional complete resection were 90.9% and 90.9% versus 61.2% and 30.6%, respectively. All factors of TNM stage correlated well with survival of patients, while the adverse prognostic effect from extent of tumor and nodal metastasis was offset by complete resection. Recurrence after complete resection occurred as a loco-regional pattern rather than dissemination or hematogenous metastasis. Conclusions: Favorable survival may be expected by extended complete resection of advanced gallbladder cancer if it is suitably indicated. (Korean J Gastroenterol 1997; 30:379-389) |
