Zusammenfassung:To evaluate the diagnostic value of lipids in the causes of ascites, 35 patients with ascites (19 with liver cirrhosis, 9 with malignant neoplasm and 6 with both liver cirrhosis and hepatocelfular car- cinomal were analyzed. The results were as follows: 1) The median value of cholesterol (18.60 +- 10.05mg/dl), triglycerides (25.00 +- 20.83mg/dl), and protein (0.910 +- 0.542 g/dl) were significantly higher in malignant ascites compared to ascitic fluid concentration of cholesterol (77.76 +- 28.60 mg/dl), triglycerides (54.89 +- 20.21 mg/dl) and protein (3.222 +- 0.707mg/dl) in patients with cirrhosis. Applying the discrimination value of 35mg/dl for cholesterol, sensitiviy, specificity and efficacy were 100%, 95%, 96.5% respectively. 2) The ascitic concentration of cholesterol (22.50 +- 19.72 mg/dl), triglyerides (17.00 +- 10.51 mg/dl) and protein (1.933 +- 1.209 mg/dl) in the cirrhosis patient accompanying hepatocellular carcinoma were not different to those of the cirrhosis patient without hepatocellular carcinoma. In conclusion, the level of cholesterol in ascitic fluids was the valuable parameter for differential diagnosis of the cause of ascites.
