Zusammenfassung: | Purpose: To develop abdominal injury score that can be used to predict the risk of abdominal injury in children after blunt trauma. Materials and Methods: Presenting clinical features and outcomes, including the need for surgery, were reviewed for 38 children who underwent computed tomography(CT) after blunt abdominal trauma. Abdominal injury score were divided into three groups: (a) low-risk group(abdominal injury score26). Higher scores denoted a progressively higher risk. CT findings were divided into two categories: (a) normal finding or minor injury, (b) major injury. Results: The low-risk group(n=12), abdomial injury score 14, had 1 patient(8.3%) with major abdominal injury and received conservative treatment. The intermediate-risk group(n=13), abdominal injury score=15-26, had 5 patients(38.4%) with major abdominal injury and among them, 1 patient with multiple abdominal injuries(liver and spleen). The high- risk group(n=13), abdominal injury score>26, had 8 patients(61.5%) with major abdominal injury and among them, 2 patients(15.4%) with multiple abdominal injuries. Three patients(23%) required urgent laparotomy and two patients(15.4%) were dead. The high-risk group, abdominal score>26, had significant high frequency of major abdominal injury. Both required laparotomy and were dead, in high-risk group. Conclusion: The abdominal injury score shows promise as a useful tool for triage of children considered for ernergency CT after blunt abdominal trauma. |
