REPAIR OF CIRCUMSCRIBED PERIAPICAL LESION SUGGESTIVE OF PERIAPICAL GRANULOMA AFTER ONE YEAR OF CLINICAL AND RADIOGRAPHIC FOLLOW-UP/REPARO DE LESAO PERIAPICAL CIRCUNSCRITA SUGESTIVA DE GRANULOMA PERIAPICAL APOS UM ANO DE PROSERVACAO CLINICA E RADIOGRAFICA/REPARACION DE LESION PERIAPICAL CIRCUNSCRITA SUGESTIVA DE GRANULOMA PERIAPICAL DESPUES DE UN ANO DE SEGUIMIENTO CLINICO Y RADIOGRAFICO

This study reported the repair of a circumscribed periapical lesion with a diagnosis suggestive of chronic periapical periodontitis. A 29-year-old male patient was referred for endodontic treatment of the upper premolar. Clinically, he was asymptomatic and had a negative response to the cold pulp sensitivity test performed with refrigerant gas. After anesthesia, the coronal opening and root canal exploration were performed. The working length was determined with an apex locator, followed by gentle irrigation with sodium hypochlorite. The canal was prepared with the Prodesign Logic 2, 35.5 system and foraminal patency was achieved with a Glide 25.1 file in the apical foramen. The root canal system was obturated using the HB single cone technique, associated with Bio C Sealer cement. Clinical and radiographic follow-up was performed one year after the canal obturation, showing repair of the periapical bone radiolucency. It is concluded that the success of endodontic therapy is achieved when the diagnosis, sanitation of the canal system and hermetic obturation are correctly performed, as well as the definitive sealing of the coronary access. Este estudo relatou o reparo de uma lesao periapical circunscrita com diagnostico sugestivo de periodontite periapical cronica. Paciente do sexo masculino de 29 anos de idade foi encaminhado para realizar endodontico do pre-molar superior. Clinicamente, apresenta-se assintomatico, resposta negativa ao teste de sensibilidade pulpar a frio realizado com gas refrigerante. Apos anestesia, foi realizada a abertura coronaria e a exploracao do canal radicular. O comprimento de trabalho foi determinado com localizador apical, seguido por irrigacao suave com hipoclorito de sodio. O preparo do canal foi realizado com sistema Prodesign Logic 2, 35.5 e a patencia foraminal foi feita com a lima Glide 25.1 no forame apical. A obturacao do sistema de canais foi realizada pela tecnica do cone unico HB, associado ao cimento Bio C Sealer. A proservacao clinica e radiografica foi realizada apos um ano da obturacao do canal, evidenciando-se reparo da radiotransparencia ossea periapical. Conclui-se que o sucesso da terapia endodontica e alcancado quando se realiza corretamente o diagnostico, saneamento do sistema de canais e uma obturacao hermetica, bem como o selamento definitivo do acesso coronario.
