Zusammenfassung:Introduction: The hepatic myxoma is a rare benign tumor that arises from the hepatic conjunctive tissue. Its pathophysiology is still poorly understood, consisting in a combination of stromal cells and a matrix of mucopolysaccharides. This case report aims to describe its clinical presentation, diagnosis and therapeutic approaches, contributing with acknowledgement for medical practice and science. Case report: A 65-year- old female patient presented abdominal pain for 30 days. CT scan revealed solid colitic diverticula and a hepatic injury in segment IV. The MRI indicated a low aggressive 3,9 cm hepatic injury. Anatomopathological examination confirmed the hepatic myxoma with no need for surgical approach. The pain ceased with modification in the eating habits of the patient. Discussion: Despite its benignancy, hepatic myxoma can present risks depending on characteristics such as size and location. In this case, due to the difficulty to surgically access the injury location, the etiological confirmation and evaluation about the surgical necessity were performed by biopsy. The diagnosis were incidentally found in the image exams, as the symptoms were nonspecific and associated with diverticular disease, association reinforced by the improvement in the pain with the dietary changes. Conclusion: Thus it is crucial for physicians to consider hepatic myxoma in the differential diagnosis of atypical hepatic symptoms. Keywords: Myxoma; liver diseases; liver neoplasms; liver. Introducao: O mixoma hepatico e um tumor raro e benigno originado do tecido conjuntivo hepatico. Sua fisiopatologia e pouco compreendida, sendo composto por celulas estromais e matriz de mucopolissacarideos. Este relato de caso visa descrever sua apresentacao clinica, diagnostico e tratamento, contribuindo com informacoes para a pratica medica e cientifica. Apresentacao do Caso: Paciente feminina, 65 anos, apresentou dor abdominal difusa por 30 dias. A tomografia revelou diverticulos colicos e uma lesao hepatica no segmento IV. A ressonancia indicou lesao hepatica de 3,9 cm com baixa agressividade. O diagnostico de mixoma hepatico foi confirmado por exame anatomopatologico, sem necessidade de tratamento cirurgico. A dor melhorou com dieta e mudancas nos habitos alimentares. Discussao: O mixoma hepatico, apesar de benigno, pode apresentar riscos dependendo de caracteristicas como tamanho e localizacao.
