Zusammenfassung:Methods, systems, and computer-readable media are provided for determining a post-acute care setting that is best suited to meet a patient's medical needs upon the patient being discharged from an acute-care facility. A set of propensity scores is generated for the patient using healthcare variables in the patient's electronic medical records. Each propensity score in the set corresponds to a particular post-acute care setting. The propensity scores may be used in association with a calculated risk of readmission for the patient to determine a post-acute care setting optimized to meet the patient's medical needs.
