Zusammenfassung:A hospital discharge preparation system is applicable to a patient and includes a high-demand case assessment interface unit for inputting data showing whether the patient is disabled, has a long-term care status, has care needs after discharge, and has specialist needs; and a case admission assessment unit electrically connected to the high-demand case assessment interface unit, wherein when the data input to the high-demand case assessment interface unit shows that the patient at least is disabled or has a long-term care status, care needs after discharge, and specialist needs, the case admission assessment unit automatically determines that the patient is eligible for admission.
