Zusammenfassung:	The present invention is related to a polymer for capturing or separating leukocyte. The polymer is prepared by a polymerized reaction using an amide-hydroxy-containing monomer. The amide-hydroxy-containing monomer has a structure of formula (1):. In formula (1), R1 is independently selected from the group consisting of hydrogen, methyl group, ethyl group, hydroxy group, one of from C1 to C12 carbon chain and benzene group, R2 is independently selected from the group consisting of hydrogen, methyl group, ethyl group, one of from C1 to C6 carbon chain, amine group and bezene group, and n is an integer ranging from 1 to 5.
