Zusammenfassung: | FIELD: medicine; particularly, transplantology. SUBSTANCE: invention is based on improvement of traditional technology of clinical transplantation of parenchymatous organs on vascular-duct crus where deliberately included into removed organocomplex from donor organism are the main own associated units on postganglionic connections and their supplying vessels and by surgical reefferention of transplant neuroganglions by recipient perivascular vegetative plexuses and implantation reinnervation of the transplanted organ by terminals of recipient nerves after formation of traditional abastomosis in zone of transplantation, partial preservation of own nervous system of graft is attained and effective restoration of its innervation lost as a result of removal from the donor organism. All this considerably increases structural-functional full-value of transplanted organ, its acute rejection is prevented under conditions of use of considerably reduced doses of immunosuppressing preparations, and terms of full-value crisisless functioning of transplant increase to reduce number of post-transplantation complications and to improve recipient life. The method is developed and realized in the course of experimental transplantation of kidneys, pancreas, lungs and liver of 96 mongrel dogs. Possibility of technical realization on human organs is tested and demonstrated in the process of imitation transplantations of kidneys and pancreas on 29 corpses of men and women. Clinical approbation is effected in the center of kidney transplantation on 11 recipients with terminal kidney insufficiency prolonged by hemodialysis. EFFECT: higher structural-functional full-valve of transplanted organ and satisfactory results of clinical transplantation of parenchymatous organs due to effective restoration of their innervation.
