Zusammenfassung:FIELD: medicine.SUBSTANCE: invention relates to the field of medicine, namely to plastic surgery and otorhinolaryngology. Soft tissues are infiltrated around the auricular cartilage. The boundaries of the excision of the skin are then marked on the back surface of the auricle in the area of the modelled antihelix. The application points are marked at three levels of quilting suture on the anterior surface of the auricle: at the point of connection of the lateral crus with the helix, at the level of separation of the formed antihelix into cruses, and at the base thereof. A strip of skin with underlying soft tissues with a width 6 mm and a length 25 mm is then excised on the posterior surface of the auricle in the area of the formed antihelix, following the marking. The skin and soft tissues are then separated on both sides by 4 mm. The cartilage is excised in the area of connection of the auricular concha with the antihelix in the form of a crescent-shaped strip with a width 4 mm and a length corresponding to the diameter of the concha. Then the cartilaginous surfaces of the auricular concha and antihelix are brought together with interrupted sutures. Next, injection needles with methylene blue are passed through the cartilage at the marked points on three levels to apply quilting sutures on the skin of the anterior surface of the auricle. The points of application of quilting sutures are then marked on three levels on the posterior surface of the cartilage at punctures made by injection needles. A puncture is then made on the posterior surface of the cartilage in the area of the inferior edge of the formed antihelix. A tunnel is formed through said puncture on the anterior surface of the cartilage, separating the skin of the anterior surface of the auricle together with the perichondrium. Then the anterior surface of the cartilage is thinned with a thin curved rasp file through the puncture on the posterior surface of the cartilage in the area of the inferior edge of the formed antihelix. Quilting sutures are then applied at three levels, wherein the quilting sutures are passed through the entire thickness of the cartilage at three levels: at the point of connection of the lateral crus with the helix, at the level of separation of the antihelix into cruses, and at the base thereof.
