Zusammenfassung:FIELD: medicine, surgery, transplantology. ^ SUBSTANCE: the present innovation could be applied in treating insulin-resistant diabetes mellitus. So, it is necessary to form venous anastomosis of inferior mesenteric vein and transplant's splenic vein at the level of horizontal duodenal branch. Transplant's arterial revascularization should be fulfilled due to anastomosing distal end of splenic artery and a recipient's left colonic artery by "end-to-end" type. Moreover, proximal end of splenic artery of the transplant should be ligated. The innovation enables to level the risk of acute disorder in circulation of left half of large intestine and its necrosis in post-surgical period, due to keeping vascular arcades between the systems of sigmoid artery and left colonic artery, achieve maximally high localization of the transplant, obtain physiological drainage horizontal position in pancreatic segment that decreases the risk of post-surgical complications and improves pancreatic function in post-transplantation period. Moreover, horizontal position of pancreatic transplant enables to adequately cover it with sigmoid flexure mesentery and isolate against surrounding tissues that decreases the risk of post-surgical complications and accelerates pancreatic reinnervation. ^ EFFECT: higher efficiency. ^ 1 dwg, 1 ex
