Zusammenfassung:FIELD: medicine, reconstructive surgery. ^ SUBSTANCE: on surface fascia in projection of supplying vessels of an autotransplant under preparation it is necessary to form the beds as separate canals for pre-sampled cartilaginous fragments at keeping septa between them together with perforated vessels; in the middle part of cartilaginous fragments one should fulfill wedge-like resection by keeping one of the surfaces, then due to applying sutures one should bring together cartilaginous edges in area of resection by forming the arched transplants; into the beds developed it is necessary to apply transplants with convex kept surface being towards the fascia, and with concave one - towards skin and the wound should be sutured in; during the second stage of operation it is necessary to take a free revascularized autotransplant as a single unit with cartilaginous fragments after a 4-wk-long interval, then cartilaginous fragments and connective-tissue capsule around them should be fixed with sutures towards autotransplant's skin, as for the autotransplant, it should be placed into the defect followed by fixing with sutures cartilaginous fragments being the constituents of the autotransplant and connective-tissue capsule around them towards either tracheal and/or laryngeal cartilages. Application of the present innovation enables to improve functional results and decrease surgical risk. ^ EFFECT: higher efficiency of surgical treatment. ^ 10 dwg, 1 ex
