MONOLITHIC MULTI-FOCUS LIGHT SOURCE DEVICE

A light source device comprising an optical device assembly and a monolithic lens is disclosed. The optical device assembly includes: a first substrate which has upper and lower surfaces facing each other; a plurality of cavities which are formed in the upper surface; and a plurality of light source chips each of which is at least partially disposed in one of the plurality of cavities and includes a light emitting device and an electrical contact; and a plurality of electrodes each of which is extended between the upper surface and the lower surface and is electrically connected to one of the electrical contacts. The monolithic lens is disposed on the upper surface of the first substrate and includes a single substrate having a plurality of lens segments, each of which is disposed on one of the light source chips.
