An apparatus for painting the inside and outside of a large-diameter pipe according to the present invention comprises: a work cart provided to be able to run on the inner or outer peripheral surface of a large-diameter pipe; a plurality of traveling rollers provided on the left and right sides of the work cart and rotatably provided in a state of being attached to the inner or outer circumferential surface of the large-diameter pipe by magnetic force; a traveling driving unit provided on the left and right sides of the work cart to drive the rotation of the traveling roller; an impurity removing unit having a rotating brush which is rotatably provided in front of the work cart and is provided to be in contact with or spaced apart from the inner or outer circumferential surface of the large-diameter pipe to remove and clean the surface impurities when being in contact with the inner or outer circumferential surface of the large-diameter pipe; and a paint spraying unit provided to be able to reciprocate continuously from side to side at the rear of the work cart and spraying paint toward an inner or outer circumferential surface of a large-diameter pipe in contact with the traveling roller while the work cart is running to perform a painting process.
