A device and an apparatus for forming a terminal are provided to form a spring unit of the terminal more accurately including an elastic unit which is extended toward a reverse direction in parallel with an extension direction of a base. A lower holder(20) comprises a lower retainer(24). An upper holder(120) comprises an upper retainer(122). The upper retainer is installed at the upward of the lower holder. The first bend die(32) and the first bend punch(132) are installed at the location corresponding to of the upper retainer and lower retainer. And the first bend die and the first bend punch are bent with regard to the first bend portion and the second bend portion of a spring unit at a predetermined angle. The second bend die(36) and the second bend punch(138) are installed at the location corresponding to of the upper retainer and lower retainer. And the second bend die and the second bend punch bend the second bent portion at a predetermined angle about an elastic unit.
