Zusammenfassung:	PURPOSE:To hold an offset form and rising reinforcements with a holding member which serves as an offset form receiving member and a reinforcement support member and enhance the mounting accuracy of the offset form and the rising reinforcement when holding the offset form and the rising reinforcement during concrete placement. CONSTITUTION:A level adjustment means 4, which adjusts the position of a level surface of an offset form 1 in a state which connects and fixes rising reinforcements 2 and the upper part of a rising surface 1a of an offset form 1 is welded and fixed with a holding member 3 which is provided with a bolt hole 3c which is a fixing means with a board surface of a semi-PC floor board B. The position of the horizontal surface 1b is adjusted with the level control means 4 and its adjustment is supported on the holding member 3. Reinforcing the rods are welded and fixed with a level adjustment nut and the rising reinforcement 2. The holding member 3 is fixed with the surface of the semi-PC floor board B with the bolt hole 3c and an embedded nut 11 of the semi-PC floor board B.
