MEDICAL SUPPORT SYSTEM AND MEDICAL SUPPORT METHOD

PROBLEM TO BE SOLVED: To provide a medical support system and a medical support method that allow both a person associated with a person in need of support and medical care practitioners to easily, quickly and correctly determine a health status of the person in need of support, and that can realize a district general care system in which nursing and medical care are properly coordinated.SOLUTION: In a medical support device 1, a point calculation section 12 calculates points indicating a health status of a care receiver, by digitizing and grading health information data acquired by a health information acquisition section 11 according to preset evaluation items. A common language conversion section 13 converts the points calculated by the point calculation section 12 and indicating the health status of the care receiver, to a common language being weather forecast expressions serving as indicators of the health status and permitting common recognition of the health status among both nursing care staff and medical care practitioners. A hospitalization necessity determination section 14 compares the points calculated by the point calculation section 12 and indicating the health status of the care receiver, to a predetermined reference value set in advance, and determines that hospitalization is required when the points are equal to the reference value or higher.SELECTED DRAWING: Figure 4 【課題】支援が必要な者と関連のある者と医療従事者との双方が、支援が必要な者の健康状態を容易、迅速、正確に判断することができ、介護と医療とを好適に連携した地域包括ケアシステムを実現することが可能な医療支援システム及び医療支援方法を提供する。【解決手段】医療支援装置１において、点数算出部１２は、健康情報取得部１１により取得した健康情報データを予め設定した評価項目にしたがって数値化して採点し、被介護者の健康状態を示す点数を算出する。共通言語変換部１３は、点数算出部１２により算出した被介護者の健康状態を示す点数を、健康状態を示す指標であって介護従事者と医療従事者の双方において共通の認識が可能な天気予報の表現である共通言語に変換する。入院要否判定部１４は、点数算出部１２により算出した被介護者の健康状態を示す点数を、予め設定した所定の基準値と比較して、その基準値以上のときに入院の必要性有りと判定する。【選択図】図４
