PROBLEM TO BE SOLVED: To provide an index sheet in which an index tab with strength high enough to be prevented from being easily torn can be formed in various positions, and an envelope.SOLUTION: An index sheet includes: a first folding piece 11 that is one folding piece formed by being folded along a fold 13 parallel to one side of a rectangular sheet; a second folding piece 12 that is the other folding piece formed by being folded along the fold 13; and a plurality of cut parts 111 in which a perforation 116 is provided in an index tab shape to have an end point on the fold 13, in the first and second folding pieces 11 and 12, and which are juxtaposed along the fold 13.SELECTED DRAWING: Figure 1B 【課題】一つのシートの様々な位置に簡単に千切れない強度のインデックスタブを形成できるインデックスシート及び封筒を提供する。【解決手段】矩形状のシートの一辺と平行な折り目１３で折り畳まれて形成される一方の折片である第一折片１１と、折り目１３で折り畳まれて形成される他方の折片である第二折片１２と、第一折片１１又は第二折片１２において、折り目１３上に端点を有するようインデックスタブ形状にミシン目１１６が設けられ、折り目１３に沿って複数並設される切込部１１１とを備える。【選択図】図１Ｂ
