Zusammenfassung:	PROBLEM TO BE SOLVED: To provide an image reading device capable of suitably suppressing density unevenness in a main scanning direction of a read image, and an image forming apparatus.SOLUTION: An image reading device 150 reads an image on a document surface by optically scanning the document surface by irradiation means 190 (constituted of a lamp 191, a reflection board 192, and a light source holding member 210 in the present embodiment) held by a moving body such as a first carriage 162, while moving the moving body, and includes inclination adjustment means (constituted of a spacer 205 held between the light source holding member 210 and an adjuster 204 in the present embodiment) for adjusting inclination of the irradiation means 190 relative to the document surface.
