PROBLEM TO BE SOLVED: To solve the problem that an information processing apparatus is required which creates a form electronic document obtained by digitizing an original which has entry data in an entry field and is sealed, in a best format and the entry data are registered in a database.SOLUTION: In the case where entry data are entered in an entry field of a non-entered form electronic document, a file name is recorded in a file name area and a first form electronic document is generated. That is printed and sealed to create an original. A document acquisition section 260 acquires a second form electronic document from an image in a file name area of original image data generated by reading the original. A collation section 270 collates the original image data with the second form electronic document. If matched, entry data of the second form electronic document are registered in a database, a seal image of the original image data is inserted into the second form electronic document, and an original form electronic document is generated. The original form electronic document has accurate contents and a small file size, tampering of the original can be detected, access control can be performed and the original can be restored.
