PROBLEM TO BE SOLVED: To provide an image forming apparatus, an image forming method, and a program, which can restrain an inspection omission of a toner patch in toner-density adjustment and suppress a memory usage amount to a minimum.SOLUTION: An image forming apparatus forms plural patches with different densities on an image carrier, and adjusts density for use in forming an image on the basis of the result of detecting the densities of the patches. The image forming apparatus pre-sets a threshold based on the densities of the patches for each formed patch, combines the predetermined number of density values, of the density values lower than the pre-set thresholds corresponding to the patches, holds the result of the combination each time the density of a single patch is detected when continuously detecting the entirety of the plural patches as a single zone, and adjusts a density based on the combined and held density values.
