Zusammenfassung:	PROBLEM TO BE SOLVED: To provide a data input system to an electronic form, allowing easy acquisition of the electronic form to be used, and allowing easy input of data to the acquired electronic form. SOLUTION: A two-dimensional code is photographed in a user terminal 10, and a user ID and a URL of a form server 40 are transmitted to a processing server 20 by use of recorded information (S1). The processing server 20 extracts an application form URL from a received request (S2, S3), thereafter accesses the form server 40, and acquires an application form (S4-S6). The processing server 20 acquires personal information by the user ID extracted from the received request (S7, S8), inputs the personal information to the application form, and thereafter transmits it to the user terminal 10 (S9).
