PROBLEM TO BE SOLVED: To suppress the lowering of a yield by comparatively reducing the number of members such as a movable reflecting mirror. SOLUTION: The optical switch device is configured so as to comprise ingoing light deflecting parts 31 to 34 capable of outputting the ingoing light at two or more different angles on a same plane as deflected light, collimated light output parts 21 to 24 capable of outputting the deflected light from the ingoing light deflecting parts 31 to 34 as collimated light passing through any of the optical axes displaced in parallel with the deflected light from the ingoing deflecting part 31 to 34 according to the different angles, and output directional path guiding parts 41 to 44, 51 to 54 for guiding the collimated light from the collimated light output parts 21 to 24 to the output directional paths according to the optical axes.
