Zusammenfassung:	PROBLEM TO BE SOLVED: To improve the reliability of medical treatment by preventing a medical chart from being fraudulently altered. SOLUTION: This access history management system is provided with: an electronic medical chart server, which is installed inside a medical facility and stores the medical charts recorded with information on patients; and an electronic medical chart alteration history manaement server, which is installed outside the medical facility, receives the notification of the registration of new medical charts in the electrical medical chart server or the reference of the electronic medical charts and/or the addition, alteration and deletion of the contents of the electronic medical charts through a network system and successively records each of the notified information. Thus, conditions of the alteration, reference or the like of the medical charts is clearly recorded, thereby preventing the medical chart from being fraudulently altered, and facilitating the cause unfolding of the medical accident to enhance the reliability of medical treatment.
