Zusammenfassung:	PROBLEM TO BE SOLVED: To provide an embedded form for concrete capable of realizing the complete integration of the embedded form and concrete, and having a dislocation retraining mechanism against a force applied in a push-out direction. SOLUTION: This embedded form 21 is a long panel provided with a plurality of ribs lined up in the push-out direction to form projecting parts 22 and groove parts 23 alternately in parallel. Circular holes 24 are bored in the bottom face of the groove part 23, and lump-like bodies 25 are provided at the side face of the projecting part and projected into the grooves.
