Zusammenfassung:	PROBLEM TO BE SOLVED: To provide a document preparation system for electronic charts wherein retrieval formulas for medical documents to be referred according to medical status are registered by medical status in advance, and necessary retrieval formulas are selected from the registered retrieval formulas and are executed, and thereby medical documents to be referred according to medical status are displayed effectively and accurately. SOLUTION: The document preparation system comprises a medical file in which medical information is recorded, a retrieval means for retrieving personal specific information from the medical file, and a display for displaying the result of the retrieval. In this document preparation system, retrieval formulas are formed in combination of a plurality of retrieval instructions, are created every medical status, and are registered in the medical file, and some of the retrieval formulas are selected according to medical status and then the retrieval means are operated to create personal medical documents according to medical status.
