PROBLEM TO BE SOLVED: To provide a concrete test piece forming concrete form and a method for manufacture of the concrete test piece using the form wherein a highly precise concrete test piece can be easily manufactured at a low cost by using a compressed air gun almost permanent by provided at a construction site or the like. SOLUTION: A concrete test piece forming form comprises a bottomed cylinder of an almost recessed sectional shape wherein a compressed air feeding hole is opened to a bottom part, a spacer part formed to an inner bottom part of the bottomed cylinder, and a bottom flat board which is horizontally fixed to an upper surface of the spacer part and formed to a slightly smaller than an inner diameter of the bottomed cylinder.
