A surgical instrument comprises handle portion; and second portion having substantial structure in three dimensions, distal region (58), and needle portion (60) having portion that is sized and shaped to extend between incision adjacent the patients obturator foramen and vaginal incision. An independent claim is also included for a surgical assembly for treating incontinence comprising surgical instrument having handle portion, needle portion having substantial structure in three dimensions and distal region, and implantable material and sheath situated on the implantable synthetic material.
