The invention provides a metasurface direct modulation MIMO transmitter with out-of-band radiation suppression capability. The metasurface direct modulation MIMO transmitter comprises a digital baseband signal module, a digital-to-analog converter, a radio frequency local oscillator module, a mixer, a horn antenna and an M * N unit metasurface. According to the invention, M * N paths of user signals are mapped into M * N paths of independent control signals by using a digital baseband signal module, phases of M * N metasurface units are independently controlled, and information is transmitted by phases of reflected waves or transmitted waves; the digital baseband signal module generates a periodic time domain window function waveform which is in periodic synchronization with a control signal code element, and the waveform passes through the digital-to-analog converter, is mixed with a carrier wave of the radio frequency local oscillator module and then is incident to the metasurface, so that the purpose of su
