The invention provides a grouting hole device based on small duct grouting and a construction method. The grouting hole device includes group-s small ducts, group-q small ducts and a water conduit. The group-s small ducts are arranged around a roadway, and one end of the group-s small ducts extends around from the inner wall of the roadway to the other end. The group-q small ducts are interlaced with the group-s small ducts, and the group-q small ducts are longer than the group-s small ducts. The water conduit is arranged in the lower part of the left side of the roadway, and is opened towardsthe right side of the roadway. As the small duct grouting construction method is adopted, small duct grouting can be carried out synchronously while high-pressure deep hole grouting is carried out. High-pressure deep hole grouting is carried out in three rounds, and small duct grouting is carried out in the two groups of small ducts alternately, so that faults with water permeability can be treated well. By using the gro
