The invention provides an optical transmission system and a manufacture method. The method includes the steps as following: a first optical waveguide structure, a first assembling pedestal and a second assembling pedestal are arranged in a print circuit board, wherein the first optical waveguide structure is positioned between the first assembling pedestal and the second assembling pedestal; an emission terminal module including an second optical waveguide structure and a first reflection surface are assembled on the first assembling pedestal, and the second optical waveguide structure is corresponding to the first terminal of the first optical waveguide structure; and the print circuit is electrically connected with the emission terminal module and a receiving terminal module in a hit-line manner, and the optical transmission system is finished.
