Zusammenfassung:Disclosed is a small optical waveguide device that produces little optical return and is provided with: an optical waveguide element in which a first output waveguide is formed at an angle to an output end face and a second output waveguide is formed at an angle to both the first output waveguide and the aforementioned output end face; and a lens that parallelizes the light emitted from the first and second output waveguides.
