Zusammenfassung: | Purpose. To study the regularities of structural and functional status of visual system of patients after surgery for retinal detachment. Material and methods. There were examined 23 patients with subtotal and total retinal detachment with macular involvement to reveal the causes of retinal functional failure after surgery. The OCT of central fundus segments, ultrasound dopplerography of posterior pole vessels, total and rhythmic electroretinography, visual evoked potentials, and central color computer perimetry were made in all patients. Results. It was revealed, that the retinal detachment is accompanied by a blood flow decrease in posterior pole vessels. The circlage procedure stimulates following decrease of blood flow resulting in formation of artificial retinal ischemia and edema. With time the blood flow improves, the normal topography of central fundus segments restores due to resorption of residual subretinal fluid. It promotes finally the visual function improvement. Conclusion. Rehabilitation of patients associated not only with a restoration of normal topography of central fundus sites, but also with a gradual improvement of regional blood flow and electrical retinal activity. It is important that М- and L-cones are activated only 3 months after surgery what can explained the maximum improvement of visual acuity in this period.Thus, the restoration of structural and functional correlations in posterior segment after surgical treatment of rhegmatogenous retinal detachment is a complex many-sided process and requires the future researches. |
