Zusammenfassung:	•We report a case of a brain abscess caused by Nocardia farcinica.•The patient had completed chemotherapy for diffuse large B cell lymphoma 2 months before infection.•Initial imaging was unable to differentiate infection vs disease recurrence.•Biopsy and diagnosis were delayed by the patient's comorbidities and goals of care.•Clinical suspicion for Nocardia merits expeditious diagnostic evaluation.
