Zusammenfassung:Highlights • Our paper describes the development and adoption of a novel model of inpatient diabetes care, expanding the role of diabetes educators to include case management, and establishment of a diabetes resource nurse program aimed at increasing the knowledge of staff nurses in caring for patients with diabetes. Retrospective analysis of readmission data was performed, showing significant reductions in readmission rates for patients exposed to this program. This program is relatively easy to adopt and was well accepted by nurses. • We believe that our paper provides data that are relevant for both clinicians and administrators examining the models of inpatient diabetes care.
