Zusammenfassung:Appendicitis is a pathology characterized by acute inflammation of the vermiform appendix and is one of the most frequent surgical emergencies worldwide, which occurs in approximately 7% of people throughout their lives. It is classified as uncomplicated, that which presents as inflammation without the existence of phlegmon, gangrene, free purulent fluid or abscess, unlike the complicated one that is accompanied by periappendicular phlegmon with or without perforation, gangrene or abscess. Currently, the use of the neutrophil-lymphocyte ratio as a biomarker for the differential severity of appendicitis has been proposed. Materials and methods: For the realization of this review article, bibliographic reviews, meta-analyses and observational studies, case series in Spanish and English languages, published in indexed journals of the last 5 years, which rest in the most important online academic databases, such as: Clinical Key, Pubmed, Science Direct, Springer Link, discarding all unconfirmed information or outdated. Results: The diagnosis of acute appendicitis is made through a clinical analysis that is complemented by a laboratory and imaging study. Scores have been established for some years to increase the sensitivity and specificity of pathology detection, it is here that the neutrophil/lymphocyte index emerges as a biomarker with acceptable sensitivity and specificity to initially detect complicated appendicitis versus noncomplicated appendicitis. complicated, this relationship can be compromised or biased in the case of the presence of other pre-existing pathologies in the patient. La apendicitis es una patología caracterizada por la inflamación aguda del apéndice vermiforme y es una de las emergencias quirúrgicas más frecuentes a nivel mundial, la cual se presenta en aproximadamente el 7% de las personas a lo largo de su vida. Se clasifica en no complicada, aquella que se presenta como inflamación sin que exista flemón, gangrena, liquido purulento libre o absceso, a diferencia de la complicada que se acompaña de flemón periapendicular con o sin perforación, gangrena o absceso.
