Zusammenfassung: | Malignant cardiac tumors and in particular primary cardiac lymphomas are very rare. Its diagnosis is usually late, which worsens the evolution and prognosis. We present the case of a patient with non-specific cardiovascular symptoms, in whom was detected, by echocardiography, a large tumor-like mass that caused severe obstruction of the right ventricular inlet tract. After performing an endomyocardial biopsy, a high-malignant lymphoma of large B-cell origin of
the heartwas diagnosed. Patient presented with cardiogenic shock,emergency cardiovascular surgery was performed with almost total unblocking of the tricuspid valve successfully achieved. Despite a favorable initial clinical course, the patient died with generalized sepsis.
Los tumores cardiaco malignos y en particularel linfoma cardiaco primario son muy raros. Su diagnóstico suele ser tardío lo cual empeora la evolución y el pronóstico. Se presenta el caso de una paciente con síntomas cardiovasculares inespecíficos, a quien se le detecta por ecocardiografía gran masa de aspecto tumoral que ocasionaba severa obstrucción del tracto de entrada del ventrículo derecho. Tras practicar biopsia endomiocárdica se diagnostica
linfoma de alta malignidad células maduras grandes origen B del corazón. Se realiza cirugía cardiovascular de urgencia al presentar shock cardiogénico y se logra exitosamente desobstrucción casi total de la válvula tricúspide. A pesar de una evolución clínica inicial favorable la paciente fallece con sepsis generalizada. |
