Sampling Procedures: A broad range of recruitment strategies were adopted including recruitment through media, venues, events, community organisations, social groups and through participants in other relevant studies; distribution of project newsletters and cards; referrals from medical practitioners; personal networking; and ''snowballing'' through friends of participants.
