Zusammenfassung: | Two cases of primary tracheal tumor ablated by endoscopic Nd-YAG laser prior to tracheal sleeve resection were reported. The first case was a 65-year-old female who complained of hemosputum and dyspnea. Fiberoptic bronchoscope revealed a tumorous lesion in the lower trachea which occluded about 75 % of the lumen, and biopsy of the tumor demonstrated an adenoid cystic carcinoma. Tracheal sleeve resection about 3.2 cm long was performed after endoscopic laser ablation, and the trachea was repaired by an inverted telescopic suture. The second case was a 51-year-old male complaining of dyspnea on effort, who had been treated under a diagnosis of bronchial asthma for six months. Fiberoptic bronchoscope revealed a smooth surfaced polypoid lesion which obstructed about 80% of the airway lumen of the trachea. Surgical resection with reanastomosis was done after endoscopic Nd-YAG laser therapy. The pathological examination revealed a pleomorphic adenoma. In both cases, median sternotomy was selected for the surgical approach, and the neck was fixed for one week by tying the jaw to the anterior chest wall in order to reduce excessive tension to the anastomosis. |
