Zusammenfassung: | A 21-year-old male patient presented with a combination of fever, palpitations, a maculopapular rash, and pharyngitis. Laboratory tests revealed an elevated high-sensitivity troponin I concentration of 3000 ng/L, indicating potential cardiac involvement. A transthoracic echocardiogram demonstrated a reduction in global longitudinal strain, suggesting impaired cardiac function. Further investigation with cardiac magnetic resonance imaging confirmed the diagnosis of acute myocarditis. After a thorough clinical evaluation, the patient was diagnosed with adult-onset Stills disease. Treatment was promptly initiated, consisting of intravenous corticosteroids and Kineret (anakinra), aimed at managing the inflammatory response and preventing further complications. |
