Zusammenfassung:Abstract Background Diffuse neuroendocrine system (DNS) and cell proliferation determines the most important pathogenetic mechanisms of non-specific ulcerative colitis (NUC) onset and course. Deepening the understanding of these processes can be an important factor for NUC onset predicting and course. Methods A total of 120 patients with NUC and 24 practically healthy (a comparison group) were examined. All patients with NUC were divided into three equal groups, depending on the severity of the disease. All patients underwent biopsy of the mucosa in the middle third of large intestine sigmoid region. Proliferative activity of the cells was determined by proliferative indices (proliferating cell nuclear antigen index-IPCNA, Cycline D1 index-ICYCL-D1) by the formula: IPCNA (or ICYCL-D1, respectively) (%) = N (the number of immunopositive nuclei to PCNA or Cyclin D1)/N (total number of cores) × 100, where N is the number of nuclei per 1 mm2 of the shear area. The death of cells in the form of apoptosis was determined from the apoptosis index (IAPT) by the formula (IAPT (%) = N (number of apoptotic nuclei stained by Mozer method)/N (total number of nuclei)immunoturburbimetry100. Immunohistochemical method was used to verify certain types of endocrine cells–apodocytes. Сommercial antibodies to chromogranin A (CgA) (Dako, 1:50), serotonin (Dianova, 1:100), melatonin (MLT) (CID Res.Inc., 1:200), vasoactive intestinal peptide (VIP) (Dako, 1:100) were used as primary antibodies. Mathematical model creation was based on regression analysis. We determined mean values, error, and confidence interval when processing the material. We took into account the diagnostic model regression coefficients and the significance of the factors by the Student's criterion. Results A comparative analysis of the whole apodocytes population indices, enterochromaffine and mast cells during the NUC exacerbation indicated a significant increase in the total endocrine cell population, a hyperplasia of endocrine cells (EC) producing MLT, serotonin, a decrease in the number of VIP secreting cells, and a decrease in the total number of mast cells. The number of apodocytes producing melatonin was maximal in mild form of NUC and in case of severe form it was minimal. The onset of NUC is associated with a sharp decrease in the proliferative activity markers and apoptosis increase.
