Cushing's disease (pituitary-dependent adrenal hyperplasia) continues to challenge the most discerning clinicians. Delineating the cause and treating the source of the hypercortisolism are often difficult, particularly in patients who have relatively indolent tumors that mimic the pituitary production of ACTH. We recently cared for a patient with Cushing's disease in whom an incidental finding of a sphenoid nodule on CT scan of the sella turcica proved to be an ectopic pituitary adenoma. Removal of the sphenoid tumor cured the hypercortisolism.
