Zusammenfassung:•Ruptured sinus Valsalva aneurysm could be an embolic source of ischemic stroke.•Acute ischemic stroke in patients with ruptured sinus Valsalva aneurysm can be managed by intravenous thrombolysis with relative safety. Sinus Valsalva aneurysms (SVA) are rare asymptomatic cardiac anomalies, which can rupture and cause heart failure, myocardial infarction and also, they can be a potential source for embolic strokes. We report the first case of a patient with acute ischemic stroke associated with a ruptured SVA, who was treated with intravenous thrombolysis (tPA) without further complications.
