5–fluorouracilによる高アンモニア血症と乳酸アシドーシスに対し持続的血液濾過透析が有効であった1例(A case of 5–fluorouracil–related hyperammonemia and lactic acidosis treated using continuous hemodiafiltration)

要旨 83歳の男性。直腸癌・多発肺転移に対してmFOLFOX6+bevacizumab療法を施行した。施行3日目に意識障害を認め，頭部CTとMRIを施行したが異常所見を認めなかった。血液検査で高アンモニア血症と乳酸アシドーシスを認め，5–fluorouracil（5–FU）の副作用と診断して持続的血液濾過透析（continuous hemodiafiltration: CHDF）を施行した。治療後は経時的にアンモニア値，乳酸値，意識レベルは改善した。5–FUを投与する際に意識障害を認めた場合は，高アンモニア血症を留意すべきである。また，5–FUによる著明な高アンモニア血症や乳酸アシドーシスに対して，CHDFが有効である可能性がある。 ABSTRACT An 83–year–old man with the diagnosis of rectal cancer and lung metastasis was treated with modified FOLFOX6 and bevacizumab. He experienced a disturbance of consciousness on treatment day 3. Computed tomography and magnetic resonance imaging study disclosed no abnormal findings in the central nervous system. Based on the laboratory data, hyperammonemia and lactic acidosis induced by 5–fluorouracil (5–FU) was diagnosed. The patient was treated with continuous hemodiafiltration (CHDF). After treatment, the level of plasma ammonia and lactic acid normalized, and the symptoms of encephalopathy resolved. Hyperammonemia is a possible side–effect of treatment with 5–FU. CHDF may be effective against the high levels of plasma ammonia and lactic acidosis in such cases.
