Vega and Rui Fernandes Failures in Jaw Reconstructive Surgery with Autogenous Onlay Bone Grafts for Pre-implant Purposes: Incidence, Prevention and Management of Complications 1 Matteo Chiapasco and Marco Zaniboni Dental rehabilitation with oral implants has become a routine treatment modality in the last decades, with reliable long-term results. However, insufficient bone volume or unfavorable intermaxillary relationships may render implant placement impossible or incorrect from a functional and esthetic viewpoint. Among the different methods for the reconstruction of deficient alveolar ridges, the use of autogenous bone blocks represents the most frequently used treatment modality both for limited and extended bone defects. Prerequisites for a successful outcome are represented by accurate preoperative planning, proper reconstructive procedure, and adequate prosthetic rehabilitation. Even if all these principles are followed, complications involving the grafts may occur, such as dehiscence, infection, or relevant resorption of the graft. The aim of this article is to present an updated overview on the inci- dence, prevention and treatment of these complications. Reoperative Orbital Trauma: Management of Posttraumatic Enophthalmos and Aberrant Eye Position 17 Simon Holmes Enophthalmos is a complex and unpredictable condition to treat secondarily, and this is likely to remain a difficult challenge. Modern imaging technology and the aggressive stance taken on appropriate primary repair make it likely that surgeons will see fewer minor cases and increased numbers of major cases. The choice of re- constructive material should be evidence-based rather than based on surgical pref- erence. Of crucial importance to the management of all traumas, particularly in revisional surgery, is attention to the soft tissue envelope, which adds to the postop- erative result, and may camouflage minor degrees of enophthalmos. Reoperative Midface Trauma 31 Robin S.Yang, Andrew R. Salama, and John F. Caccamese Reoperative midface surgery can be challenging. Although well-established surgical principles are still the basis of surgical approaches and techniques, the advent of new materials and technologies brings about opportunities to achieve the best pos- sible outcomes with bony reconstruction and more precise results. Soft tissue defor- mities continue to be some of the most challenging, especially as they relate to the orbit, but continually evolving techniques offer improved results for volume correc- tions to treat enophthalmos and diplopia. Conventional orthognathic and recon- structive rhinoplasty techniques can also be applied to great effect and with satisfying results to treat posttraumatic malocclusions and nasal deformities. Reoperative Mandibular Trauma: Management of Posttraumatic Mandibular Deformities 47 Luis G. Vega Mandibular fractures are one the most common maxillofacial injuries. Diagnostic errors, poor surgical technique, healing disorders, or complications may lead to the establishment of posttraumatic mandibular deformities. Nonunion, malunion/ malocclusion, or facial asymmetry can be found early during the healing process or as long-term sequelae after the initial mandibular fracture repair. Although occa- sionally these problems can be solved in a nonsurgical manner, reoperations play an important role in the management of these untoward outcomes. This article dis- cusses the reoperative techniques used for the management of these deformities. Reoperative Soft Tissue Trauma 63 Tirbod Fattahi Trauma remains the leading cause of death in the first 4 decades of life and is sur- passed only by cancer and atherosclerotic disease as the overall leading causes of death in the United States. Many of the injuries involve the facial region, including soft tissue trauma. This article highlights the current available modalities used in the management of unsightly scars or those scars whose location and appearance compromise function. Reoperative Orthognathic Surgery 73 Johan P. Reyneke The best time to perform orthognathic surgery for the correction of dentofacial deformities is the first time. However, complications requiring reoperation do occur. A thorough understanding of how to avoid intra- and postoperative complications, and how to manage these problems successfully, is mandatory. This article dis- cusses some of the most common complications, how to avoid these complications, and how to treat complications when they do occur. General surgical complications during and after surgery, such as hemorrhage and infection, are outside the scope of this article. Avoiding Revision Rhinoplasty 93 Peter D. Waite Reoperation of the nose is challenging and sometimes emotionally difficult for the surgeon and patient. There are multiple pitfalls to be avoided and it is always best to carefully diagnose and establish a surgical treatment plan. Even among the best of plans and surgical techniques, revision may be necessary. The patient and surgeon should understand the limitations of the surgical techniques and the individ- ual anatomy. Considerations in Revision Rhinoplasty: Lessons Learned 101 Tirbod Fattahi This article is intended to deal with the difficult subject matter of revision rhinoplasty. Since there is consensus that rhinoplasty is one of the most difficult aesthetic sur- gery procedures, one would make the inference that a revision rhinoplasty should also be one of the more difficult revision surgeries. The intent of this article is to share with the readers a few pearls and lessons learned dealing with revision rhinoplasty. Reoperative Face and Neck Lifts 109 Jacob Haiavy The complexity of the reoperative facelift or neck lift is directly related to the way the primary procedure was performed. Regardless of the primary technique used, the secondary procedure should be directed to the specific problems that the patient exhibits, such as scars, earlobe deformity, hair pattern changes, laxity in the upper face or neck, jowling, or deepening of the nasolabial folds. Contour should be re- stored within the deep layer support via the elevation of the superficial musculoapo- neurotic system and platysma rather than rotating skin flaps in an exaggerated manner in a cephalad direction, producing a tight unnatural look. Reoperative Temporomandibular Joint Surgery 119 Luis G. Vega, Rajesh Gutta, and Patrick Louis TMJ surgeries are not always successful. Many potential pitfalls can occur during any phase of the treatment and can lead to complications, less than desirable re- sults, and short- or long-term failures. Unsatisfactory results can occur for multiple reasons, including misdiagnosis of the original pathologic condition, incorrect selec- tion of surgical technique, technical failures, complications, systemic disease, and unrealistic expectations. This article focuses on the reoperation of the TMJ primarily in cases of internal derangement and discusses TMJ arthrocentesis, arthroscopy, modified condylotomy, and open joint procedures. Reoperative Midface Reconstruction 133 Julio Acero and Eloy Garcia Reoperative reconstruction of the midface is a challenging issue because of the complexity of this region and the severity of the aesthetic and functional sequela re- lated to the absence or failure of a primary reconstruction. The different situations that can lead to the indication of a reoperative reconstructive procedure after previ- ous oncologic ablative procedures in the midface are reviewed. Surgical techniques, anatomic problems, and limitations affecting the reoperative reconstruction in this region of the head and neck are discussed. Reoperative Mandibular Reconstruction 153 Phil Pirgousis, Nathan Eberle, and Rui Fernandes Ideal reconstruction of the mandible is important for a multitude of reasons and has been and continues to be among the most common surgical challenges for recon- structive surgeons of the head and neck. Historically, pedicle flaps, such as the pec- toralis major and deltopectoral myocutaneous flaps, were workhorse flaps for lower facial third head and neck reconstruction. This article outlines the relevant anatomy of the perimandibular region, reconstructive options including second free flaps, rel- evant workup, and complications pertaining to reoperative mandibular surgery. Reoperative Maxillofacial Oncology 161 Daniel Petrisorand Rui Fernandes Approximately 36,000 people in the United States will be diagnosed with oral or oro- pharyngeal cancer in 2010. In more than 90% of the cases of oral cavity and oropha- ryngeal cancers, histopathologic examination reveals squamous cell carcinoma (SCCA). Despite appropriate initial treatment, oral SCCA recurs in 25% to 48% of cases. Several studies have shown that most cancers recur within about 2 years af- ter the initial treatment. Hence, close follow-up of patients is important for the timely detection of recurrences.This article discusses the types of recurrence and the sur- veillance and treatment of recurrent oral SCCA. Reoperations in Cleft Lip and Cleft Palate Treatment 169 Rafael Ruiz-Rodnguez and Juan Carlos Löpez-Noriega The surgical management of cleft lip and palate is a difficult and complex endeavor. Several surgical techniques for the treatment of this deformity have been described around the world; each one, when properly done by expert surgeons, renders good and predictable results most of the times. However, the fact that there are so many techniques means that there is no universal procedure that will always deliver great esthetic and functional results. This article discusses the causes of inadequate results in primary cleft lip and palate surgery and describes the various secondary surgical techniques to correct the same. Reoperative Treatment of Obstructive Sleep Apnea 177 Mansoor Madani, Farideh M. Madani, and Dmitry Peysakhov Over the last several decades, various surgical treatments have been advocated and used to treat obstructive sleep apnea. Treatments ranging from aggressive proce- dures, such as tracheostomy, to the least-invasive procedures, such as radioabla- tion, have not yielded satisfactory results. One of the major causes for surgical failures and lower success rates lies in the inadequate understanding and appreci- ation of the anatomic and pathophysiologic factors that contribute to upper airway obstruction. In some cases, combinations of various surgical techniques may help improve the conditions. This article reviews several major types of surgical proce- dures, their complications, and the recommended approaches for retreatments. Index 189
